Controlled Medication Refills
To ensure safe prescribing and comply with state and federal regulations, controlled medications cannot be refilled through our online refill request form.
This includes medications such as:
- Testosterone
- ADHD medications (such as Adderall)
- Any other controlled substance prescribed by your Forge provider

How to Request a Controlled Medication Refill
Simply schedule a 15-minute Virtual Refill Appointment through our online scheduler.
During your appointment, your provider will:
- Review how you're doing on your current treatment
- Discuss any side effects or concerns
- Make dosage adjustments if needed
- Approve and process your refill
The appointment fee is $150 or 150 Forge Points.
If you're an active Forge Member and have at least 150 Forge Points available, you can use your points to cover the visit. If you don't have enough points, the standard $150 appointment fee will apply.
Non-Controlled Medication Refills
Most non-controlled medications, peptides, and supplements do not require a follow-up appointment if your treatment plan is stable.
Simply complete the appropriate Medication Refill Request Form Below. Our medical team will review your request and process your refill if no additional evaluation is needed.
If your provider determines that your medication needs to be adjusted or your condition requires further discussion, we'll ask you to schedule a 15-minute Virtual Follow-Up Appointment before your refill can be approved.
